Our office provides a multiple disciplinary approach to health care where our team of physicians works together to help you get out of pain.

How Do I Schedule an Appointment?

You can just call our office or fill out the contact us form and we will call you to schedule your appointment.

What Do I Need for My First Visit?

General: Insurance Card, Driver’s License, Referral (if needed), Paperwork (if completed at home), Diagnostic results (X-ray, MRI, CT Scan, Lab work etc..)

If you were involved in an auto accident or work injury the adjuster, case manager, claim information, and/or attorney information would also be needed.

Please wear comfortable or loose fitting clothing to allow for the examination of your injury.

Will It Hurt?

The examination and/or treatment is not meant to cause harm but to find the source of your pain or help determine the right treatment options for you.

Is It Covered By Insurance?

Most insurance will provide coverage for care but it is determined by the limits of your policy.  If there is something that is not covered by insurance or your do not have insurance, there are other payment options available.

What Do I Need To Pay?

Your insurance policy co-pay, co-insurance, or deductible would need to be paid at the time of your visit.

How Long Is My Appointment?

Most appointments will be about a hour long.

What Treatment Will I Need?

Your physician will discuss your treatment options with you and you will have a good understanding of the next steps on your road to recovery.
